The Center for Healthy Hearts
The Center for Healthy Hearts has operated as a safety-net free clinic in the greater Richmond area since 1983. Currently, the center serves as a specialty medical home to more than 500 uninsured patients with chronic diseases including hypertension, diabetes, dyslipidemia, and heart failure. This pharmacist-led clinic works in partnership with volunteer physicians under a collaborative practice agreement.
The success of this model was recognized in 2016 by the Community Pharmacy Foundation as one of 10 best practices in "Get the Medications Right: A Nationwide Snapshot of Expert Practices." A detailed description of the practice was also published in Pharmacotherapy in 2016. In addition to these publications, the center was also highlighted at the 2016 Emswiller Interprofessional Symposium and the “Innovative Practices in Billing for Patient Care Services” session at the 2017 APhA Annual Meeting.
